TERMS & CONDITIONS

FORM ONE
Client Limited Power of Attorney Only
For The Purpose of Credit Repair

This form is a limited power attorney and is used only to help you with the repair and restoration of your credit. This does not give us full power of attorney over your affairs it's only for use with credit concerns and can be cancelled at any time.
Client will here after be referred to as "Person or People" who have sent this form and have agreed to the terms and conditions by checking the "I agree button" below.
CLIENT give and appoint Credit Clean a div. of WebHead Ventures, Inc, it's officers, employees and agents, located at 532 Bridle Path Way, Tarpon Springs, Florida 34688, phone (727) 939-0311 as my assistant for my behalf, as set forth in the following matters only. Signing of correspondence addressed to the credit bureaus, signing of correspondence addressed to creditors, obtaining credit information over the telephone, fax, through written correspondence from credit bureaus, creditors or collection agencies. We will research and verify all negative accounts to find the best way to resolve these issues. I give Credit Clean a div. of WebHead Ventures, Inc, it's officers, employees and agents the right to discuss information to help resolve a debts listed on my credit files with the three major credit bureaus Equifax, Trans Union and Experian, or any other credit reporting agency. I further authorize Credit Clean a div. of WebHead Ventures, Inc, its agents, servants and employees bearing this release or copy thereof to obtain any information in my credit report that may involve medical records and/or credit records. I hereby direct said record holder to release such information upon the request of the bearer of the authorization to release information and durable Power of Attorney for limited purposes. I hereby release the bearer of this authorization as well as the recipient, including but not limited to, the Custodian of such records, Repository of the Court Records, Credit Bureau, (TRW, Trans Union, Equifax, Experian) consumer reporting agency, retail business establishment, lending institution, student loan agencies (public and/or private), including whatever kind, which may at any time result to me, my heirs, family or associates, because of compliance with this Authorization to release Information, or any attempt to comply with it. Should there be any questions as to the validity of this Release, you may contact me as indicated below. I have the right to revoke or terminate this Power at any time. All questions pertaining to validity, interpretation and administration of the Power shall be determined in accordance with the laws of Florida. This power is valid throughout the United States in all information set forth in the paragraph above by signatures below clicking the (“I agree button below”)
Note to Consumer: I know that I may cancel this Power of Attorney at any time by submitting a letter stating I no longer give power of attorney to your office. This power of attorney can only be used for credit issues and will not be used for anything other than stated above.

FORM 2
Client Service Agreement

Client hereinafter referred to as "client" and Credit Clean a Division of WebHead Ventures, Inc. Hereinafter referred to as "Credit Clean", located at 532 Bridle Path Way, Tarpon Springs, Fl 34688, phone (727) 939-0311, agree as follows:

1. Scope of Services: the purpose and scope to this agreement is that Client and Credit Clean have entered into a Client Service Agreement. Credit Clean will assist Client in the QUICK CLEAN Credit Repair Program. Client must obtain a copy of all three credit reports from one of the approved locations noted on Credit Clean's website. Client must send by email, fax or submission the forms required to being the Quick Clean Process. Client must also send a copy of the required credit reports or give instructions on how to get a copy of Client’s credit reports from each of the three bureaus (Equifax, Trans Union and Experian). Client must send required Proof as defined on the Quick Clean page (proof of address, proof of name and proof of social) Client may fax, mail or email this proof. Client must make the payment required on the Quick Clean page for either a Single Account $385 or Joint Account $585. Once the proper reports have been obtained by Credit Clean an evaluation and analysis of these reports will be made for the purpose of finding derogatory information affecting the credit scores. Client should note all accounts that are negative and give any information about each account that could help with the clean up process. Even if the account belongs to you and is true it can still be disputed and possibly removed so the more information you can give the better. Credit Clean, as requested by client, will be acting as a Counselor and Client authorizes Credit Clean to fully assist in the process of removing obsolete, incorrect, misleading, inaccurate, unknown and unverifiable information from each credit report that is not in compliance with the Federal Fair Credit Reporting Act, Fair Debt Collection Practices Act and any other set guidelines for creditors, credit bureaus and collection agencies. Credit Clean services are to include, but not limited to, education of client in reading credit reports, counseling client and informing client of his/her rights under the Fair Credit Reporting Act and the Equal Credit Opportunity Act. Credit Clean acts as Clients personal assistant for the process of Credit Restoration under the QUICK CLEAN PROCESS for a total of three months or three rounds of disputes. The Quick Clean Process is a three month or three dispute program which ends after the third dispute round. If three disputes have not been made by the end of the third month Credit Clean will work until there have been three rounds of disputes processed. At such time as the third round of disputes has been processed Credit Clean has fulfilled the requirements of the Quick Clean Program. Credit Clean is paid by Client for such services that assist client with the restoration of his/her creditworthiness. Credit Clean cannot by law guarantee any results from the work performed. Credit Clean uses ten years of knowledge and experience to help client get the best reports and scores possible. The fee is a one time fee; no other money is required unless Client enrolls in another program. Client has the opportunity to cease using the services of Credit Cleans QUICK CLEAN program at any time but once the payment for the Quick Clean Service has been made Credit Clean cannot and will not refund any monies paid for this service. You may cancel your service with three business days of sending your paper work to Credit Clean’s office. The Quick Clean Service is to help those who have minor credit problems obtain a higher score and better reports, quickly. It is not a quicker way to get normal credit work done and is the decision of the client to use this Quick Clean process as opposed to the Full Service Program. Credit Clean will provide the best possible help and will use every means available to help the Client repair and restore credit reports in and effort to establish creditworthiness and higher credit scores. Client agrees to be responsible for all cost required to obtain consumers credit reports from the approved places listed on Credit Clean's Web page. Any other type of credit report cannot be used for this program. This Quick Clean program consists of setting up and disputing negative items on each the three credit reports. Giving client answers to any credit questions he/she may have with the ultimate goal of raising client credit scores and improving client credit reports. There are three months of work built in to the Quick Clean Program or three rounds of disputes. If three rounds of disputes are not obtained in the three months of work then Credit Clean will work until all three rounds are completed as long as this work is not delayed by the client due to delayed sending of credit reports client has received. The QUICK CLEAN program does encompass all three major credit bureaus and the reports by all three credit bureaus are worked on individually. This process usually takes three months of hard work although improvements are most likely seen within 60 days. You pay Credit Clean only the FLAT rate shown for this service and nothing more unless you convert to another program … at which time a new agreement will be signed. This agreement is for the QUICK CLEAN PROGRAM only and no other statements other than what are contained here are applicable to the QUICK CLEAN PROGRAM. Credit Bureaus are now offering you the option to obtain credit scores, credit scores are not included with this service and are best obtained by the client since the cost for Credit Clean to order a score is much higher than what they can be obtained for online by the consumer. Client can obtain credit reports and scores at the same time. The Quick Clean program does not provide for reports client must pay for these at the time service begins.

Important Notation:
WE DO NOT CHARGE YOU FOR PHONE CALLS like other companies do. We would, however, appreciate it if you use email as much as possible to save valuable time your counselor needs doing the tedious and stressful work that needs to be done By emailing questions your counselor also has a log of questions, answers and concerns to refer back to when working on your credit files. Client Understands Credit reports are regulated by the Credit Reporting Bureaus and Client is allowed by law to receive one credit report per bureau per year when denied credit because of information obtained by said credit bureau.
The FLAT RATE fee charged for the Quick Clean Service is the total amount to be paid and no other fees apply. The payment MUST be made at the time client uses the service. Credit Clean will not be held responsible for any articles that become lost or delayed in the mail. Client must notify Credit Clean if Client changes residences as credit bureaus DO NOT FORWARD credit reports and as such Credit Clean cannot use the address sent in at the time client began service and must have new proof of address sent immediately. Credit Clean will re-order any reports that are missing within a normal period of time. Please be patient with this service it takes time and results are well worth the wait. 2. Client authorizes Credit Clean, its employees and its servants to prepare all necessary correspondence utilized in the processing of disputes and agree to submit to Credit Clean any additional information required to support legally the dispute process. Client understands that all disputes must be true and accurate and that Credit Clean will not produce any information that is not accurate to the best of Clients knowledge. Credit Clean will request from credit bureaus and/or creditors, collection agencies and attorneys, verification of all information being reported negatively by the credit reporting agencies and will use every legal means of removing negative data from Clients reports so that Client can restore a more positive credit history and increase credit scores in an effort to get better interest rates and loans. Credit Clean does not change or advise Client to change Clients Identification, social security number or any other personal data in an effort to gain credit under a different identity. This is illegal and you should not use companies who offer this type of service. 3. Client agrees to the FLAT FEE payment schedule for the Quick Clean Credit Repair Program and that ONE TIME PAYMENT means the account is paid in full. This Quick Clean Program is designed to help those with minor credit problems HOWEVER client must be aware that this is NOT a faster way of cleaning credit this is a quick way of fighting a few problems on credit reports with an end date and flat fee paid.
STATE LAW REQUIRES US TO GIVE A 5 DAY NOTICE OF CANCELLATION AND FEDERAL LAW A 3 DAY NOTICE OF CANCELLATION, Credit Clean does not have any restriction on cancellation, you can cancel our service at any time we have no long term binding agreement and ends when three disputes processes have been done which is normally about three months. If client cancels service within the first three to five days client will be responsible for the cost of the work incurred during the three to five day period of time. A refund minus the cost of service fee will be sent to client within thirty days from the date service is cancelled.
Quick Clean Credit Services are a flat fee of $385.00 for a single account or $585 for a joint account. This is a service for those who have few credit problems and need to get the scores up as high as possible in a 90 day period of time. We will work the ninety days for the flat fee with no other monies owed. Do not choose this program if you have bad reports it is going to end up costing you more if the work isn't done in the ninety days.
You agreed to these terms and conditions of the "QUICK CLEAN Client Service Agreement Form 3" when clicking the I AGREE BUTTON.

Please Read This It is Required That We Inform You of Your Rights

You have a right to dispute inaccurate information in your credit report by contacting the credit bureau directly. However, neither you nor any "credit repair" company or credit repair organization has the right to have accurate, current, and verifiable information removed from your credit report. The credit bureau must remove accurate, negative information from your report only if it is over 7 years old. Bankruptcy information can be reported to 10 years. You have a right to obtain a copy of your credit report from a credit bureau. You may be charged a reasonable fee. There is no fee, however, if you have been turned down for credit, employment, insurance, or a rental dwelling because of information in your credit report within the preceding 60 days. The credit bureau must provide someone to help you interpret the information in your credit file. You are entitled to receive a free copy of your credit report if you are unemployed and intend to apply for employment in the next 60 days, if you are a recipient of public welfare assistance, or if you have reason to believe that there is inaccurate information in your credit report due to fraud. You have a right to sue a credit repair organization that violates the Credit Repair Organization Act. This law prohibits deceptive practices by credit repair organizations. You have the right to cancel your contract with any credit repair organization for any reason within 3 business days from the date you signed it. Credit bureaus are required to follow reasonable procedures to ensure that the information they report is accurate. However, mistakes may occur. You may, on your own, notify a credit bureau in writing that you dispute the accuracy of information in your credit file. The credit bureau must then reinvestigate and modify or remove inaccurate or incomplete information. The credit bureau may not charge any fee for this service. Any pertinent information and copies of all documents you have concerning an error should be given to the credit bureau.
If the credit bureau's reinvestigation does not resolve the dispute to your satisfaction, you may send a brief statement to the credit bureau, to be kept in your file, explaining why you think the record is inaccurate. The credit bureau must include a summary of your statement about disputed information with any report it issues about you. The Federal Trade Commission regulates credit bureaus and credit repair organizations. For more information contact:

TIPS:
1. You should keep a copy of all items sent to Credit Clean so as to have a back up in case anything happens to your mail coming to you or to us.
2. Make sure to send us all the proof of address, social and name we need.
3. Make sure to either make your payment online on via regular mail at the time you want to begin the service.
4. If you have any questions please call us.
5. Clear your spam for email coming from creditclean.com or tampabay.rr.com as these are the two ISP we use for our email to clients. If you don’t clear these domains you may never get our email especially if you use an account such as HOTMAIL, YAHOO or MSN etc. These companies have spam built in to the email and the only way you can receive our email is to clear the names above or to go looking for it in your junk mail.
6. When you get reports send them in … don’t wait even if all the reports have not come in yet. We can work on what you have while waiting for missing reports.
7. If in doubt ask your counselor about anything you intend on doing during this work.
 

 
Section Three - PROOF NEEDED
Retain for your records

IMPORTANT: Make sure to clear your spam protection to accept email from the domain CreditClean.Com email will come from different areas of Credit Clean so in order to make sure you get them you need to stop your computer from sending our mail to the junk mail file.

1.  Proof of Address: You need (2) items below and they must show the address you want mail to be sent to.  This must be the address you want your credit reports and other information to go to.  You can use a P.O. Box or a Physical Address.  Be careful using a business address, the address you send proof of is the address they will list on your credit reports as you home address. Proof can be any of the following as long as it has the address you want to use and you have two of six items you can use as proof.

  • Driver's License

  • Major Credit Statement

  • Voided Check

  • Lease Agreement

  • Utility Bill

  • State I.D.

You must send two of the above proof of address and if you are a joint account you must send two of the above proof of address for each person.
 

2.  Proof of Social Security Number: You only need (1) of these. This can be proven with any of the following:

  • Social Security Card

  • Filed Tax Returns

  • Employer Pay Stub

You only need to send one of the above but if you are joint account both need to send proof.

Explanation:

  • Driver's License:  Or state I.D. You should send a copy of your Driver's License even if it does not have your current address.  It will show proof of your previous address, date of birth and driver's license number.  Some states use your Social Security Number as your Driver's License Number.
     

  • If you do not send these things we will not be able to do any work as the bureaus will not respond to anything we do without proof of the items below.  All the items above will cover all of these things.

    • Proof of your current address

    • Proof of your previous address

    • Proof of you birth date

    • Proof of your Driver's License number

    • Proof of your social security number
    • Proof of age

Just make sure you have something that proves each one of these and send them to us immediately by fax or mail.  If you are not a client and you are trying to get your reports you must also make sure to enclosed the proof needed above.

You are now allowed ONE FREE report from each credit bureau per year by going to:
www.annualcreditreport.com or by calling 1-877-322-8228. You will only get one (FREE) report from each bureau per year.  Ordering just one credit bureau will leave you two other bureau reports for later on during the year or you can order all three at the same time. You won't be able to get another free report for a full year. If you want to pay the credit bureaus money you can get as many reports as you want to ... it gets expensive though so try and use what you can get for free.

NOTE:  If you are using Credit Clean's Quick Clean Credit Repair Service you will need to get your reports and send them to us.  Thank you

  • If you order your credit reports by mail it will take approximately 2 to 4 weeks to receive them and you will need to follow the instructions on what to include with your request for a credit report from each bureau.
    If you order your reports by phone it will take approximately five days to seven days to receive them.  They will come by mail so be careful not to throw them out as junk mail this is exactly what they look like.  There are three bureaus you will need to get three reports (one from each credit bureau) If you do not get all three you are only seeing one third of your credit.

If ordering your reports by mail you need to do the following:

1.  Type a letter stating you want a copy of your consumer credit report.
2.  List your name, address, social security number and date of birth.
3.  Sign the letter and date it.
4.  Enclose a copy of two items showing proof of your address go to: PROOF NEEDED
5.  Enclose a copy of an item that shows proof of your social security number  PROOF NEEDED
6.  We suggest you mail the requests by certified mail but it is not mandatory.
7.  For most states the cost of a credit report is $9.50 you will need to enclose a check or money order for this amount.


Section Five - Duties
RETAIN THIS FOR YOUR RECORDS
1.  When Credit Clean is doing your work the credit bureaus respond by sending an updated credit report.  These credit reports will only go to you.  The updated credit reports will come to you about once three months if you chose the Gold Monitoring Program or once every six months if you chose the Silver Monitoring Program.  Credit bureaus will not send reports to anyone but you.  Even though we are doing all the work for you the reports must get to our office in order for your counselor to know what is left to do and what has been deleted or changed on each credit report.  This is the major responsibility of you the client.  If we don't get the reports sent to us then we can't work on your credit files which delays your service.  When you send reports your file is activated for more work and given to your counselor.  Always send any and all communications you get from a credit bureau whether it be a credit report, form letter or other.  You should also send any correspondence you receive from creditors or collection agencies that are reporting negatively on your reports.  We need to see what is being done by each and every company.  You don't have to send those separately you can send them each month with your credit reports.  Monitoring reports means evaluating them so we cannot do that without the reports.

2.  Notify us if you change addresses, phone numbers or email.  This is very important since changing address could cost a month of work to be lost.  Credit bureaus do not for the most part forward credit reports.  The reason is that anyone could walk in and change your address at a post office.  Credit reports are very sensitive items and contain a lot of your personal information.  Forwarding credit reports is not something that will be done by most post offices since the credit bureaus post on the out side of the envelopes "Do Not Forward".  We must get proof of your new address and send this in to the credit bureaus in order to get updated reports going to your new address.  Please notify your counselor if moving is in your near future and also notify when the actual move occurs. We have an easy update form online at: www.creditclean.com/client_update.htm   We will then need to get PROOF OF ADDRESS from you as soon as possible.  Work shouldn't be done until we have proof to send to the credit bureaus.  You can get more information on what is needed to show proof of address on our website.

Make sure to clear any spam protection to accept anything from creditclean.com.  Sometimes you will receive email from different sections of our company and in order to make sure you receive them please check your spam protection in your email and approve or clear creditclean.com
